We will be happy to verify your insurance coverage, and to help you make affordable financial arrangements. We accept:

• Cash or check payment at time of visit
• Insurance
• Visa/MasterCard

Dr. Forde is a participating provider with Blue Cross Blue Shield and Medicare, and accepts auto and work compensation insurance and other insurance coverage.

Insurance Coverage for Massage Services:

Insurance may cover massage therapy if there is a medical necessity, and in some cases, if it is done in conjunction with chiropractic treatment.

We require a chiropractic examination to evaluate your need and will be happy to verify your insurance coverage, and to prescribe massage if your condition warrants it.

There will be a charge for missed appointments that are not canceled at least 24 hours in advance.
